  • skin / stratified squamous epithelium growing in middle ear cleft
  • probably due to chronic eustachian tube blockage leading to chronic negative pressure
  • expanding cyst fills middle ear

Examination

  • foul smelling
  • perforation
    • occupied by desquamated keratin (pearly)

Complications

  • expansion → destruction
    • ossicles
    • facial nerve
    • inner ear
  • infection
    • venous sinus thrombosis
    • extra-dural abscess
    • subdural abscess
    • meningitis
    • brain abscess

Treatment

  • surgical removal - radical mastoidectomy
