Cardiac Enzymes

  • Measured after Myocardial Infarction
  • necrosis of cardiac muscle releases enzymes into circulation
  • serial measurements

Creatinine Kinase

  • peaks within 24 hours
  • usually back to normal in 48 hours
  • sources
    • heart muscle
    • brain
    • skeletal muscle
  • different isoenzymes exist
    • CK-MB specific to heart muscle

Troponin I / Troponin T

  • released early
    • 2-4 hours
  • persist for up to seven days
  • highly specific for cardiac injury

Aspartate aminotransferase / Lactate dehydrogenase

  • non-specific
  • rarely used
  • LDH
    • peaks at 3-4 days
    • remains elevated for up to 10 days
