Epidemiology

  • present in many patients with moderate renal impairment
  • present in almost all patients with end-stage renal failure

Pathogenesis

  • decreased renal production of 1a-hydroxylase
    • → ↑ parathyroid hormone
    • → increased osteoclastic activity, cyst formation, bone marrow fibrosis
  • may lead to tertiary hyperparathyroidism
    • osteosclerosis
  • hypocalcaemia and vitamin D deficiency → osteomalacia
  • adynamic bone disease
