Ophthalmia Neonatorum


  • Chlamydia trachomatis infection in neonate
  • develops in infants around 14 days after birth
  • appears in 1/2 infants born through infected cervix
  • follicles rarely seen

Clinical Features

  • swelling of the eyelids and orbit
  • hyperaemia
  • purulent infiltration of conjunctiva
    • does not respond to chloramphenicol / cleansing

Investigation

  • conjunctival swab (after pus removed)
    • immunofluorescence

Course

  • resolves spontaneously
  • 6 weeks later
    • chlamydial pneumonia
      • mild
      • atypical
      • insidious onset
      • marked X-ray changes

Treatment

  • erythromycin 7 days
    • eliminates nasopharyngeal carriage
