Muscle Origin Insertion Nerve Action Notes
pronator teres medial epicondyle,
coracoid process
half way down front of radius median pronates forearm median nerve passes between two heads
flexor carpi radialis medial epicondyle 2nd and 3rd metacarpals median flexes and abducts wrist  
palmaris longus medial epicondyle palmar aponeurosis median flexes wrist not always present
flexor carpi ulnaris medial epicondyle,
olecranon,
posterior border of ulna
pisiform bone ulnar nerve flexes and adducts wrist  
flexor digitorum superficialis medial epicondyle,
coracoid process,
front of upper part of shaft of radius
middle phalanges of fingers median flexes fingers splits into 4 tendons before passing flexor retinaculum,
splits at phalanges to allow flexor digitorum profundus to pass through
flexor digitorum profundus medial and anterior aspects of shaft of ulna and interosseous membrane distal phalanges of fingers ulnar nerve
median
flexes distal phalanges passes through tendons of flexor digitorum superficialis
flexor pollicis longus anterior surface of radius distal phalanx of thumb median flexes thumb  
pronator quadratus distal end of shaft of ulna distal end of shaft of radius median pronates
