Prostatitis
- 25 percent of visits for men presenting for urologic evaluation of lower
urinary tract symptoms
- uncomplicated treated adequately with a 30-day course of antibiotics
History
- Acute febrile prostatitis
- Fevers
- chills
- rigors
- frequency
- urgency
- dysuria
- difficulty emptying the bladder
- acute urinary retention
Examination
- T = 104 f
- distended, tender lower abdomen
- Prostate massage should be avoided in the acutely infected patient as this
could result in severe pain, bacteremia, and unrecoverable urosepsis
Treatment
- empirically with broad spectrum agents
- hospitalisation
- Persistent spiking fevers may indicate prostatic abscess
