Principles of screening

(after Wilson and Jungner)

  1. The condition is an important health problem
  2. It's natural history is well understood
  3. There should be a recognisable early stage
  4. Treatment at an early stage should be more reliable than treatment started at a later stage
  5. There should be a suitable test
  6. The test should be acceptable
  7. There should be adequate facilities for diagnosis and treatment of abnormalities detected
  8. For diseases of insidious onset screening should be repeated at intervals determined by the natural history
  9. Physical or psychological harm should be less than the benefit
  10. The cost should be balanced against the benefit
  • should be agreed policy on whom to treat as patients

  • case-finding should be ongoing process
